Prior to recertification, it is suggested that you review your current listing in the OPA database to ensure that all information is correct, including the Authorizing Official’s email address and eligible child sites. If information on the database is found to be incorrect, it should be updated before recertification by submitting a change request to the OPA. The Electronic Change Request from can be found here and should be submitted by the Authorizing Official.
In the upcoming weeks, the Authorizing Official will receive a username and password from HRSA via email to use for recertification. Failure to recertify within the given time frame will result in removal from the program.
Only the entity’s Authorizing Official may submit for 340B recertification or change request forms. The OPA defines the Authorizing Official as “someone who can bind the organization to a contract, such as the CEO, CFO, COO, Executive Director, President or Vice President of the entity.”
